Terms & Conditions: As the signer, I agree that I represent the company/organization and am authorized to engage in the interest of the business listed. I agree that Life Exec and HEALTH365+, LLC. and its authorized vendor partners may contact myself and my company/organization for the interest of Employee Health and Welfare Benefits. ā€‹ This will confirm that we have authorized LifeExec and HEALTH365 Plus, LLC. (dba: HEALTH365+) to assist us with our employee welfare benefit programs effective immediately. HEALTH365+ is authorized to review our existing contracts and rates; and this letter constitutes your authority to furnish a HEALTH365+ representative with all information they may request as it pertains to our employee and retired employee health insurance & welfare benefits programs, contracts and past communications. We are not terminating our existing broker and they should still receive all commissions negotiated until we notify you otherwise. Our existing broker will also be responsible for the day to day service of our existing contract. Please work with HEALTH365+ as they request other lines of coverage in addition to the Health Insurance & welfare benefits that were recently placed. Negotiations on any additional lines of coverage that HEALTH365+ requests should only be discussed with a HEALTH365+ representative.

Minimum Essential Coverage

Required for ALEs

Minimum Essential Coverage (MEC) is mandatory for Applicable Large Employers (ALE), who are identified as having 50 or more full-time equivalent employees (combining both full-time employees and part-time employees' hours equivalent to full-time).

ACA Fines

According to the IRS, an Applicable Large Employer (ALE) that does not have a Minimum Essential Coverage (MEC) qualified program in place may face fines. The specific penalty amount is $247.50 per employee, calculated by subtracting 30 from the total number of Full-Time Equivalents (FTEs), and applies for each month that the ALE does not have MEC coverage.
